The patient was referred to me from another doctor for removal of a broken tool from the distal root of the upper left sixth tooth.

After breaking the tool was pushed through top foramen and got stuck in tissues outside the root.
 
     
 

Before removal of a broken tool I prepared and filled the distal root canal.

Since the tool stuck in tissues outsider the root, the only possibility to remove it was to perform a surgical procedure.

After detachment of the mucoperiosteal flap it turned out that there is no broken tool in the bone. I took another intra-procedural RVG picture in order to locate a broken tool.

It turned out that the tool was primarily stuck in soft tissues of oral cavity and after detachment of the mucoperiosteal flap it moved along with the flap.

By means of a microscope I found the broken tool in soft tissues of oral cavity and I removed it.
